Does anyone have any advice on the most effective way to set the levels using Para Acoustic DI w/ iMix Notch pickup? The pickup is in a Larrivee OM-9 and I just recently purchased the Para Acoustic DI to give the guitar more body during solo concerts. My uneducated guess would be to set the iMix EQ to "0" across the board and make my adjustments using the DI. Most of the time, I am plugging into simple PA with 15in speakers playing in gyms or church halls for groups of 100-500.

I'm sorry to say I will be unable to make effective EQ setting recommendations. The tone is affected by too many variables and you'll need to adjust as needed for each playing environment based on how it sounds to you.
The most I'm able to do is refer you to the "Setting the Gain" section of the Para DI user's guide. As you get a feel for how the EQ on the pickup system, as well as that of the Para DI, affect the sound, you'll be able to dial in a good tone more easily in most playing situations.
